Case Managers

Amanda Miotto – Registered Practical Nurse & Case Manager

RPN

Amanda Miotto is a Nurse Case Manager with Rehab First and provides case management services to clients with catastrophic injuries; including spinal cord injuries and acquired brain injuries. As a nurse case manager, Amanda works in collaboration with multidisciplinary medical and rehabilitation team members to ensure client’s rehabilitation needs are met in regards to physical, emotional and psychological care.


Amanda has experience working as a community nurse, focusing primarily on pain and symptom management and palliative care; being Fundamentals and CAPCE certified. In addition, Amanda has experience working as a Physiotherapy Assistant in both clinic and community settings.

 

Colleen Lewis – Registered Nurse & Case Manager

RN

Colleen Lewis is a Registered Nurse, who provides comprehensive case management services to clients with catastrophic impairments such as traumatic brain injuries, spinal cord injuries, chronic pain, and multiple impairments.


Colleen has been an RN for over 23 years, with an extensive background in nursing both in the US and Canada. Her professional experience includes: cardiac and critical nursing care, community nursing supervisor providing oversight to patients in the community and community health care workers, tele-health nursing supervisor providing guidance, support, training and performance evaluation to RNs, providing tele-triage to callers in a tele-health environment. Colleen has taught student RNs (Diploma and Degree Programs) and RPNs (Practical Nursing Program) as a sessional instructor in a number of colleges in the GTA., including Sheridan College, Centennial College and Humber College/University of New Brunswick. In addition, Colleen has worked in the private healthcare sector as an independent contractor, providing educational in-servicing and training on medical products to surgeons, nurses and other health care professionals. She has also provided training to administrative personnel and health care providers in long term facilities on the Nursing Restorative Services as mandated by the Ministry of Health and Long Term. Colleen has also served as a Legal Nurse Consultant. Colleen holds a Bachelor of Science Degree in Nursing from the University of Downstate in New York State. She also possesses numerous additional training and qualifications as well as on-going professional development.


Eugenia Osei – Registered Nurse & Case Manager

BScN, RN

Eugenia Osei is a Nurse Case Manager at Rehab First and has provided case management services to clients with catastrophic injuries ranging from spinal cord injuries and acquired brain injuries. She is a Registered Nurse with the College of Nurses of Ontario. She holds a Bachelor of Science in Nursing through the collaborative program between University of Western Ontario and Fanshawe College. Currently, she is working towards the Project Management Certificate through Western’s continuing studies. During her employment in hospital, Eugenia has worked primarily with an orthopaedic and trauma population, including adults with acquired brain injury, spinal cord injury, multiple trauma injury and amputations.


James Campbell – President & Director of Rehabilitation Services

Registered Psychotherapist, M.Ed., MBA, CCRC, RRP, CCLCP, CVRP(F)

James Campbell is the co-founder of Rehab First and maintains active practice in Case Management and Life Care Planning for Rehab First's most complex clients.


Jamie’s areas of specialization include vocational, educational and personal counselling and he has worked with varied client populations including children, adolescents and adults; people with brain injury, spinal cord injury, amputation, chronic pain, mental illness, addiction, learning disability and many other types of injury or illness.


James holds Master of Education degrees in Educational Psychology and Counselling Psychology from the University of Western Ontario, where he also earned his Master of Business Administration degree. He holds the Canadian Certified Rehabilitation Counsellor Certification (CCRC), Registered Rehabilitation Professional (RRP) designations and is a Canadian Certified Life Care Planner (CCLCP). Jamie is also an active member of the Canadian Association of Rehabilitation Professionals (CARP) and is a recognized authority on rehabilitation best practices.


Joanne Gram – Vice President Rehabilitation Services

M.Ed. (CRDS), OCT, RRP, CCRC, CCLCP, MCVP, CVRP(F)

Joanne Gram is the Director of Rehabilitation Services of at Rehab First Inc. She has been providing case management, vocational rehabilitation, employability assessments and future care cost services to the legal, government and insurance industries since 1981. For 16 years, she was the Vice President of Operations at Associative Rehabilitation Inc., a disability management firm that provided various rehabilitation and medical services across Canada and the United States. During this time, in addition to direct file work, she was responsible for supervising the professional staff and ensuring quality standards were met.


Subsequently, Ms. Gram was employed as Vice President of Public Relations at Aetna Health Management, a subsidiary of Aetna Canada.


Ms. Gram is an Ontario Certified Teacher (OCT) registered with both the Ontario College of Teachers, and the College of Vocational Professionals. She graduated with a Master of Education in Community Rehabilitation and Disability Studies from the University of Calgary. She has taken additional training seminars and graduate level courses, as well as presented on adult and pediatric traumatic brain injury, spinal cord injury, special education, case management, rehabilitation and counselling. Ms. Gram holds a number of professional certifications which designate her as a Canadian Certified Rehabilitation Counsellor (CCRC), a Registered Rehabilitation Professional (RRP), a Canadian Certified Life Care Planner (CCLCP). She also holds a Master’s Certificate as a Vocational Professional (MCVP).


Kelly Gillis – Registered Nurse, Case Manager and Regional Director

RN

Kelly Gillis, a Registered Nurse since 1992, is Rehab First’s Regional Director responsible for a large multi-disciplinary group spanning Central-West, Southwestern and Western Ontario. She is also the leader of the Case Management Practice, which she has developed into an acknowledged centre of excellence. In her clinical work she specializes in the delivery of case management services for clients with complex and catastrophic injuries. Kelly has worked with a range of client populations including those who have suffered from brain injuries, spinal cord injuries, multiple and aggravated injuries and amputations as well as psychological and psychosocial impairments. Kelly is a member of the National Case Management Network (NCMN). Prior to joining Rehab First, she was a charge nurse at a nursing home providing nursing care and case management services for medical-surgical, neurological impairments, and palliative, geriatric, and psychiatric patients with neurobehavioral difficulties. She also acted in a supervisory capacity. Kelly has further clinical experience in both a community-setting and within a psychiatric facility. Kelly earned her nursing diploma from Lambton College, where she was on the Dean’s Honours List for three consecutive years, the University of Western Ontario, where she earned her LeveI l and Level ll Palliative Care certification, and McMaster University where she earned her Certification in Case Management.


Laura Miller – Registered Nurse & Case Manager

RN, B.Sc. 

Laura Miller is a Registered Nurse who provides comprehensive case management services for the Bruce, Grey and Huron County regions. She acts as a single point of responsibility for clients who have suffered catastrophic injuries including brain injury, spinal cord injury and physical or psychological impairments. She coordinates services in these rural areas in a way that ensures their needs are being met in a timely and efficient manner.


Laura has an extensive background in nursing and pediatric case management. She has experience in both community-based and hospital-based care and has worked as a community nurse and Hematology Case Manager. She holds an advanced degree in Nursing from McMaster University as well as a Bachelor of Science from the University of Guelph and has been trained in the administration of the Progressive Goal Attainment Program (PGAP) for clients experiencing disability due to pain.


Nicole Dutchak – Social Worker & Regional Manager (Hamilton)

BA, BSW, MSW, RSW

Nicole Dutchak is a Regional Manager with Rehab First Inc. She is a registered social worker with the Ontario College of Social Workers and Social Service Workers as well as a member of the Ontario Association of Social Workers. Nicole has provided counselling and case management services to clients in the Hamilton, Burlington and Niagara regions since 2012. Nicole is an experienced practitioner in both community and healthcare settings. As a medical social worker, Nicole conducted psychosocial assessments, case management, and discharge planning in areas such as internal medicine, complex care, neo-natal intensive care and the emergency department. She also provided specialized assessments within a regional mental health program. As a community practitioner, Nicole worked in residential treatment settings and provided counselling, case management and recreational support to individuals who experienced severe and persistent mental health concerns. Nicole is skilled in providing individual, couple, and family counselling. She is also experienced with working in group settings. Nicole has extensive experience in providing support to individuals and families who are adjusting to injury or illness. She has particular expertise in the areas of traumatic brain injury, anxiety, depression, addictions, and pain management.


Nicole is passionate about her profession and feels privileged to have the opportunity to work with individuals who are recovering from motor vehicle accidents, occupational or non-occupational injuries or illnesses. She utilizes a client-centered approach in order to empower clients to regain their independence and look forward to the future. Nicole holds Bachelor Degrees from McMaster University in Sociology and Social Work. She holds a Master Degree in Social Work with a specialization in clinical practice, also from McMaster University. Nicole is also fluent in French.

 

Robyn Vanderhoek - Registered Nurse & Case Manager

RN 

Robyn Vanderhoek is a Nurse Case Manager at Rehab First Inc. and provides case management services to clients with catastrophic injuries such as spinal cord injuries, acquired brain injuries and amputation. She graduated from the Registered Nursing program at Seneca College in King City, ON in 2004. Robyn has gained valuable experience working as an RN in hospital and Long Term Care settings. Robyn has knowledge and understanding in many different areas of healthcare; including surgery, geriatrics, rehabilitation and management. Robyn has worked most recently as a Director of Care for a Long Term Care Home. Robyn works routinely to enhance healthcare to provide quality care.


Valerie Price – Social Worker

MSW, RSW

Valerie Price is a registered social worker with the Ontario College of Social Workers and Social Service Workers who provides personal counselling to clients in London and the surrounding area. She holds her Master of social work degree from the University of Calgary, a Bachelor's degree in social work from King’s University College (UWO) and a diploma in Workshop Rehabilitation from Humber College in Toronto. Valerie has extensive experience working with clients providing individual, couple, family, and group therapy and has worked with children, adolescents, and adults. Her clinical experience includes practice in both community and healthcare settings to support individuals within various service sectors: developmental services, child welfare, community mental health, acquired brain injury, in-home health care, and treatment and prevention of interpersonal violence.
