Amanda Miotto – Registered Practical Nurse & Case Manager
Amanda Miotto is a Nurse Case Manager with Rehab First and provides case management services to clients with catastrophic injuries; including spinal cord injuries and acquired brain injuries. As a nurse case manager, Amanda works in collaboration with multidisciplinary medical and rehabilitation team members to ensure client’s rehabilitation needs are met in regards to physical, emotional and psychological care.
Amanda has experience working as a community nurse, focusing primarily on pain and symptom management and palliative care; being Fundamentals and CAPCE certified. In addition, Amanda has experience working as a Physiotherapy Assistant in both clinic and community settings.
Anthony Ogini – Nurse Case Manager
Anthony Caines Ogini is a Nurse Case Manager at Rehab First, and has provided case management services to clients with complex and catastrophic injuries, ranging from Spinal Cord injuries and Acquired Brain injuries. As a Nurse Case Manager, he works in collaboration with multidisciplinary medical and rehabilitation team members, to ensure a proactive approach of care, is taken to meet a client's physical, emotional and psychological needs.
As a Registered Nurse, licensed with the College of Nurses of Ontario, Anthony has extensive experience in both community and critical care settings. Anthony areas of specialization include critical care, community health and mental Health. Anthony holds an Honors Bachelor of Science in Nursing through York University with a certificate in Critical Care from Humber College. Anthony is an active member of the Canadian Association of Critical Care Nurses.
Ashley MacDonald – Physiotherapist and Regional Director (Dorchester)

Ashley MacDonald is a registered physiotherapist and Regional Director. Ashley graduated from St. Francis Xavier University with a Bachelor of Science in Chemistry, then completed her Masters in Physical Therapy at the University of Western Ontario. Since that time Ashley has continued her education in manual therapy and acupuncture and has completed courses through the McKenzie Institute. Hers clinical experience includes work in an outpatient orthopedic setting, where she was responsible for assessing and treating individuals with predominantly musculoskeletal and neurological conditions. Additionally, she has experience with conditioning and work hardening programs. Ashley utilizes a combination of treatment techniques including manual therapy, soft tissue and neural mobilization, active exercise therapy and patient education to help each individual improve their mobility and restore function.
Azrah Lavji – Occupational Therapist & Regional Manager (Oshawa)

Azrah Lavji is a certified occupational therapist who is registered
with the College of Occupational Therapists of Ontario and is also a
member of the Ontario Society of Occupational Therapists. Azrah earned
her double degree consisting of a Bachelors of Science and Masters of
Science in Occupational Therapy from D’Youville College. She has also
obtained a Bachelors of Arts in Sociology from McMaster University.
Azrah is fluent in Gujarati and has clinical experience in a variety of
roles including mental health, pediatrics, orthopedic, and neurological
rehabilitation. Azrah is also certified in performing Functional
Abilities Evaluations (FAEs).
As an occupational therapist at Rehab First, Azrah provides therapy
services in Oshawa and the surrounding areas. She provides physical,
cognitive and affective assessment and treatment to enable return to
daily activities at home and at work. She improves client’s functional
independence through education, environmental modification and provision
of assistive devices. Azrah is involved in discharge planning and
collaborates with families and hospital staff to ensure a safe
transition to home. She places strong emphasis on client centered
practice, enabling clients to reach their full potential in their
self-care, productivity, and leisure occupations.
Carissa Morris – Registered Nurse & Regional Manager (Ottawa)

As a nurse case manager, Carissa works in collaboration with multidisciplinary medical and rehabilitation team members to ensure client’s rehabilitation needs are met in regards to physical, emotional and psychological care.
As a Registered Nurse, licensed with the College of Nurses of Ontario, Carissa has extensive experience in both community and acute care settings. Carissa has experience working as a community nurse, focusing primarily on pain and symptom management and palliative care. Carissa holds a Bachelor of Science in Nursing through Laurentian University with a certification in Community Health Nursing through the Canadian Nurses Association.
Carol Hawkins – Social Worker & Regional Manager (Richmond Hill)

Carol Hawkins is a Regional Manager in the Toronto area with Rehab First Inc. She is a registered Social Worker with the Ontario College of Social Workers and Social Service Workers as well as a member of the Ontario Association of Social Workers. Carol has provided case management services to clients for over 30 years throughout Ontario. Carol’s extensive experience makes her particularly skilled at handling complex cases.
Along with working directly with a wide variety of clients, Carol also possesses a management background which has allowed her to mentor other rehabilitation professionals and teach them the importance of being client centered, empathetic, non-judgemental, as well as pro-active and positive in their approach.
Carol holds a Bachelor’s degree in Social Development Studies from Waterloo University and a Bachelor of Social Work degree from the University of Windsor. Carol is always pursuing new learning and participates in numerous professional development activities and attends industry related conferences on a regular basis.
Eugenia Osei – Registered Nurse & Case Manager
Eugenia Osei is a Nurse Case Manager at Rehab First and has provided case management services to clients with catastrophic injuries ranging from spinal cord injuries and acquired brain injuries. She is a Registered Nurse with the College of Nurses of Ontario. She holds a Bachelor of Science in Nursing through the collaborative program between University of Western Ontario and Fanshawe College. Currently, she is working towards the Project Management Certificate through Western’s continuing studies. During her employment in hospital, Eugenia has worked primarily with an orthopaedic and trauma population, including adults with acquired brain injury, spinal cord injury, multiple trauma injury and amputations.
James Campbell – President & Director of Rehabilitation Services

James Campbell is the co-founder of Rehab First and maintains active practice in Case Management and Life Care Planning for Rehab First's most complex clients.
Jamie’s areas of specialization include vocational, educational and personal counselling and he has worked with varied client populations including children, adolescents and adults; people with brain injury, spinal cord injury, amputation, chronic pain, mental illness, addiction, learning disability and many other types of injury or illness.
James holds Master of Education degrees in Educational Psychology and Counselling Psychology from the University of Western Ontario, where he also earned his Master of Business Administration degree. He holds the Canadian Certified Rehabilitation Counsellor Certification (CCRC), Registered Rehabilitation Professional (RRP) designations and is a Canadian Certified Life Care Planner (CCLCP). Jamie is also an active member of the Canadian Association of Rehabilitation Professionals (CARP) and is a recognized authority on rehabilitation best practices.
Jennifer Breckels - Social Worker & Regional Manager (London)

Jennifer has a Bachelor of Sociology from Wilfred Laurier University and a Bachelor of Social Work from the University of Waterloo. Jennifer is registered and in good standing with the Ontario Association of Social Workers and the Ontario College of Social Workers and Social Service Workers. Jennifer will be providing social work assessment and treatments services to children, adolescents and adults in the London area who have been injured in motor vehicle accidents.
She has worked with an assortment of agencies including child welfare, mental health and various community organizations. Jennifer has extensive experience providing support to clients including children, adolescents and adults. She has worked collaboratively with various professionals to ensure all needs of a client are met within the larger family and community setting. Jennifer has training and experience working with diverse and disadvantaged populations, including working with people who have mental health challenges, developmental disabilities, addiction issues or are involved in the criminal justice system.
Joanne Gram – Vice President Rehabilitation Services

Joanne Gram is the Director of Rehabilitation Services of at Rehab First Inc. She has been providing case management, vocational rehabilitation, employability assessments and future care cost services to the legal, government and insurance industries since 1981. For 16 years, she was the Vice President of Operations at Associative Rehabilitation Inc., a disability management firm that provided various rehabilitation and medical services across Canada and the United States. During this time, in addition to direct file work, she was responsible for supervising the professional staff and ensuring quality standards were met.
Subsequently, Ms. Gram was employed as Vice President of Public Relations at Aetna Health Management, a subsidiary of Aetna Canada.
Ms. Gram is an Ontario Certified Teacher (OCT) registered with both the Ontario College of Teachers, and the College of Vocational Rehabilitation Professionals. She graduated with a Master of Education in Community Rehabilitation and Disability Studies from the University of Calgary. She has taken additional training seminars and graduate level courses, as well as presented on adult and pediatric traumatic brain injury, spinal cord injury, special education, case management, rehabilitation and counselling. Ms. Gram holds a number of professional certifications which designate her as a Canadian Certified Rehabilitation Counsellor (CCRC), a Registered Rehabilitation Professional (RRP), and a Canadian Certified Life Care Planner (CCLCP). She also holds the designation of Certified Vocational Rehabilitation Professional, Fellow CVRP(F).
Jobyna Bell, Registered Nurse, Case Manager
Jobyna Bell is a Nurse Case Manager at Rehab First, and has provided case management services to clients with complex and catastrophic injuries, ranging from Spinal Cord injuries and Acquired Brain injuries. As a Nurse Case Manager, she works in collaboration with multidisciplinary medical and rehabilitation team members, to ensure a proactive approach of care, is taken to meet a client's physical, emotional and psychological needs.
As a Registered Nurse, licensed with the College of Nurses of Ontario, Jobyna has extensive experience in both community and healthcare settings. Prior to joining the Rehab First team, she was a nurse in the NICU and Paediatric departments. Additionally, she has managed and supervised health care professions from both nursing and other health care disciplines, and acquired experience sitting on multiple health-related committees.
Judith Wiens – Social Worker & Regional Manager (Hamilton/Niagara-Western GTA)

Judith is registered Social Worker with the Ontario College of Social Workers and Social Service Workers. She is also a Registered Rehabilitation Professional with the Vocational Rehabilitation Association who provides counselling services at Rehab First Inc. out of the London office.
Judith provides counselling, case management, community re-integration, and vocational services for Rehab First Inc. throughout the Niagara Region. Judith has worked in community settings providing assessments and case management for a variety of clients including those with developmental, physical and intellectual disabilities. Judith brings with her more than ten years of management and supervisory experience. Judith is experienced in providing individual, couples, children/adolescent, and family counselling bringing with her extensive experience in the field of disabilities, mental health, addictions, and crisis intervention strategies. Judith graduated with a Bachelors and Masters of Social Work (BSW/MSW) from the University of Manitoba and she is proficient in American Sign Language (ASL).
Kelly Gillis – Registered Nurse, Case Manager and Regional Director
A Registered Nurse for 26 years, Kelly Gillis specializes in the delivery of Case Management services for clients who have complex injuries and catastrophic injuries.
Kelly has worked with a range of client populations, including those who have brain injuries, spinal cord injuries, multiple and aggravated injuries, amputations and psychological and psychosocial impairments. Prior to joining Rehab First, she was a nurse at a long-term care facility, and provided nursing care and case management services for patients with medical-surgical, palliative and geriatric needs, as well as those with neurological impairment and psychiatric neurobehavioral difficulties. She has managed and supervised health care professionals from both nursing and other health care disciplines and has additional clinical experience in community settings, hospitals and a psychiatric facility.
Kelly earned her nursing diploma from Lambton College, where she was on the Dean’s Honours List for three consecutive years, her Level 1 and Level 11 Palliative Care Certification from the University of Western Ontario, and her Certificate in Case Management from McMaster University. She is a member of the National Case Management Network (NCMN).In addition to providing case management at Rehab First, she is the Director of Rehabilitation Services, directly managing a multidisciplinary treatment team and supervising Regional Managers who teams provide a wide variety of assessment and treatment, vocational rehabilitation and attendant care services throughout Southwestern Ontario.
Laura Miller – Registered Nurse & Case Manager
Laura Miller is a Registered Nurse who provides comprehensive case management services for the Bruce, Grey and Huron County regions. She acts as a single point of responsibility for clients who have suffered catastrophic injuries including brain injury, spinal cord injury and physical or psychological impairments. She coordinates services in these rural areas in a way that ensures their needs are being met in a timely and efficient manner.
Laura has an extensive background in nursing and pediatric case management. She has experience in both community-based and hospital-based care and has worked as a community nurse and Hematology Case Manager. She holds an advanced degree in Nursing from McMaster University as well as a Bachelor of Science from the University of Guelph and has been trained in the administration of the Progressive Goal Attainment Program (PGAP) for clients experiencing disability due to pain.
Melissa Jamieson – Occupational Therapist and Acting Regional Director (Hamilton/Niagara-Western GTA)

Melissa Jamieson is an Occupational Therapist servicing clients in the Burlington and Hamilton areas. She earned her Joint Honours Bachelor of Health Sciences in Psychology and Dance at the University of Waterloo and her Bachelor of Health Sciences in Occupational Therapy at McMaster University. Her clinical expertise has been acquired in a variety of settings in Canada, where her work experience includes roles at the WSIB and TTC, and India, where she studied at the Christian Medical College and Hospital in Tamil Nadu. She has extensive experience in the areas of disability management, workplace health, attendant care, vehicle modifications, home modifications, and health promotion. Her scope of practice focuses on assessment and treatment of physical, cognitive, and mental health conditions to support clients’ return to independence in self-care, productivity, and leisure. Melissa is a Certified Disability Management Professional (CDMP) and has maintained this designation since 2015.
Neha Hasan-Gill – Occupational Therapist & Regional Manager (Mississauga)

Neha is an Occupational Therapist at Rehab First, registered with the
College of Occupational Therapists of Ontario. She is also a member of
the Ontario Society of Occupational Therapists. Neha attained her Master
of Science in Occupational Therapy and her Honours Bachelor of Science,
with a focus on Cell and Molecular Biology, from University of Toronto.
While obtaining her Masters, Neha gained experience in various practice
settings including neurocognitive rehabilitation after acquired brain
injuries, mental health rehabilitation, hand therapy, and complex
continuing care. At Rehab First, Neha is involved in the physical,
cognitive and affective assessment and treatment of individuals who have
been injured in motor vehicle collisions. This encompasses providing
education, recommending environment modifications, providing assistive
devices, and facilitating rehabilitation in the areas of self-care,
leisure, and productivity. Using a client-centred and collaborative
approach, Neha works with clients to help them return to occupations
that are meaningful to them.
Nettya Senthilnathan – Occupational Therapist & Regional Manager (Woodbridge)

Nettya Senthilnathan is a certified occupational therapist who is registered with the College of Occupational Therapists of Ontario and is also a member of the Ontario Society of Occupational Therapists. Nettya earned her Bachelor of Science (Honours) with a double major in Health studies and Psychology from University of Toronto and her Masters in Occupational Therapy from University of Sydney.
Nettya is fluent in both English and Tamil and has clinical experiences as an Occupational Therapist in a variety of health care settings in Canada and internationally. As an occupational therapist at Rehab First, she by provides therapy services in North York and the surrounding areas. She works with individuals who have conditions/ injuries that are mentally, physically, developmentally, or emotionally disabling, helping them to develop, recover, or maintain daily living and work skills. Nettya not only helps clients improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function, with a goal to help clients have independent, productive, and satisfying lives.
Nicole Dutchak – Social Worker, Case Manager & Regional Director

Nicole Dutchak is a Regional Manager with Rehab First Inc. She is a registered social worker with the Ontario College of Social Workers and Social Service Workers as well as a member of the Ontario Association of Social Workers. Nicole has provided counselling and case management services to clients in the Hamilton, Burlington and Niagara regions since 2012. Nicole is an experienced practitioner in both community and healthcare settings. As a medical social worker, Nicole conducted psychosocial assessments, case management, and discharge planning in areas such as internal medicine, complex care, neo-natal intensive care and the emergency department. She also provided specialized assessments within a regional mental health program. As a community practitioner, Nicole worked in residential treatment settings and provided counselling, case management and recreational support to individuals who experienced severe and persistent mental health concerns. Nicole is skilled in providing individual, couple, and family counselling. She is also experienced with working in group settings. Nicole has extensive experience in providing support to individuals and families who are adjusting to injury or illness. She has particular expertise in the areas of traumatic brain injury, anxiety, depression, addictions, and pain management.
Nicole is passionate about her profession and feels privileged to have the opportunity to work with individuals who are recovering from motor vehicle accidents, occupational or non-occupational injuries or illnesses. She utilizes a client-centered approach in order to empower clients to regain their independence and look forward to the future. Nicole holds Bachelor Degrees from McMaster University in Sociology and Social Work. She holds a Master Degree in Social Work with a specialization in clinical practice, also from McMaster University. Nicole is also fluent in French.
Shauna Smith – Occupational Therapist & Regional Manager (Brantford)

Shauna Smith is an Occupational Therapist serving clients in the Kitchener-Waterloo and Hamilton-Niagara areas. She earned her Masters of Occupational Therapy and her Bachelors of Health Sciences from the University of Western Ontario.
At Rehab First, Shauna’s scope of practice includes assessing and treating clients to improve their physical, affective and cognitive function. This encompasses the recommendation and placement of assistive devices, providing education, and facilitating rehabilitation and maintenance in the areas of self-care, leisure and vocation. She has a specific focus on return-to-work programming and vocational rehabilitation.
Stephanie Neale – Occupational Therapist & Regional Manager (Guelph)

Stephanie Neale is a registered Occupational Therapist with the College of Occupational Therapists of Ontario (COTO) and a regional manager with Rehab First Inc. She completed her Master of Science in Occupational Therapy at McMaster University and obtained a Bachelor of Science in Honours Kinesiology from the University of Waterloo. Stephanie has clinical experience in the areas of complex rehabilitation, acquired brain injury, traumatic injury, and chronic disease and is currently completing coursework to become a certified Life Care Planner. She has experience working with a range of client populations including pediatrics, adolescents, adults, and older adults. Stephanie provides assessment and treatment services for individuals with both catastrophic and non-catastrophic injuries, including discharge planning, provision and implementation of assistive devices and technology, client education, cognitive rehabilitation, life skills development, home assessment and modification, and case management. As a Regional Manager, Stephanie is responsible for supervising a team of multidisciplinary therapists in the Kitchener-Waterloo and Guelph areas.
